Use this form to assist the people you choose to write letters of recommendation for you. Complete the form, make several copies, and give one to each letter writer.

Use this form to list the scholarships that you have been offered by a school, an organization or individual. Please use the complete and official name of the scholarship and the organization that offers you a scholarship. Even though the form asks for the value (amount) of the scholarship, please know that we will never publish the amount of the scholarship. We use the value to calculate the total amount of awards given to PTHS students each year. You can turn in as many of these forms as you need to the Counseling Center. Deadline for Awards Ceremony Program: TBA
