Discipline Plan

Dear Parent/Guardian:

My main goal as your child’s teacher will be to motivate your child to learn in a positive manner. This goal cannot be attained unless all of m students support me in this effort. In order to insure your child the excellent learning climate he/she deserves, I am utilizing the following DISCIPLINE PLAN.

Classroom Rules:

1. Be responsible for classwork, and complete assignments to the best of your ability

2. Be responsible of homework, and complete assignments to the best of you ability.

3. Listen carefully, follow directions, and use self-control with your classroom behavior.

4. Follow campus hallway, cafeteria, and restroom rules.

5. Be responsible for bringing all supplies to class.

6. Follow Accelerated Reader rules.

Each student begins each six weeks with a 100 conduct grade. Each time a rule is broken, a student will receive a “sign in” on a weekly chart that I keep at my desk. A point is subtracted off of the conduct grade for each “sign in.” Each student’s conduct will be recorded daily on their “Assignment/Conduct” sheet, which is located in the back of their Take-Home Folder. The “Assignment/Conduct” sheet will need to be signed or initialed nightly, so that parents are aware of conduct grades, as well as, assignments.



Consequences for Breaking Rules:

1. For each rule broken, a student will receive a “sign in” on the conduct chart.

2. If the same behavior continues, your child will receive another “sign in” and may or may not have to write a letter home explaining what was done (depending on the situation).

3. If the behavior continues, I will make one of these three choices, depending on the situation:

* Parent/Teacher conference
* Referral to Principal or Counselor
* Student’s desk will be moved into isolation

***Severe behavior will automatically be referred to the Principal or Assistant Principal.



I want to reward those students who work hard at following the rules! Fun Friday will be offered for students who have received 3 or fewer “sign ins.” At the end of the six weeks students who have maintained a 100 conduct grade will receive a treat and a certificate for their Memory Book. Students who have maintained 95-99 conduct grade will also receive a treat.
