DPT - 5 doses unless 4th dose on or after 4th birthday. Students 7 years or older - three doses. One dose of Td required ten years after the last dose.

Polio - four doses unless the 3rd dose on or after the 4th birthday.

Measles, Mumps and Rubella series - two doses of a measles-containing vaccine with the first dose on or after the first birthday and second dose by age 5.

Varicella - One dose on or after 1st birthday. Two doses if vaccine given at 13 years of age or older.

Hepatitis B series - three doses are required.

HIB vaccine is required for children before the age of 5.

PCV7 - completed series of 4 doses with the last one 12 months or over for children attending pre kindergarten, Head Start, or any early childhood programs.

Hepatitis A - two doses separated by six to 18 months for children 2 years of age or older attending pre kindergarten, Head Start, or any other early childhood programs.
